Alabama has secured $203 million for the first budget period of a five-year grant through the Rural Health Transformation (RHT) Program, part of a historic $50 billion federal investment authorized by the One Big Beautiful Bill Act.

Alabama’s RHT Program prioritizes transformation and sustainability, using targeted investments to enable new models of care and workforce pipelines to serve rural citizens. The program’s goal is providing healthcare transformation through healthier populations, stronger workforce and local economic resilience.

What happens next?

Timely distribution of funding, per the guidance set by the Centers for Medicare & Medicaid Services, is our top priority. Once the appropriate paths for programs are defined, funding opportunities will be posted on this site.

The following initiatives are being targeted to begin the work of transforming Alabama's rural healthcare systems. 

Cancer Digital Regionalization

Strengthening cancer care coordination through connected providers, specialty access, and digital care networks that help patients receive care closer to home.

EMS Trauma & Stroke Initiative

 

Improving emergency response systems for trauma and stroke patients through faster assessment, treatment, and care coordination.

EMS Treat-in-Place Initiative

 

Supporting innovative EMS models that allow qualified providers to treat appropriate patients on-site while reducing unnecessary hospital transports.

Maternal & Fetal Health

 

Improving maternal and infant outcomes through expanded prenatal care access, specialty consultation, and coordinated support services.

Simulation Training

 

Advancing workforce development through realistic clinical simulation training that enhances provider preparedness and strengthens rural healthcare capacity.
